Full-time Life Path Support Worker
(80 hours by-weekly)
The First Nation Healing Centre is a non-profit organization which is presently seeking a Life Path Support Worker. The Life Path Support Worker will provide person-centered support to residents as they pursue meaningful, self-directed lives. You will assist clients in developing daily living skills, community involvement, independence, and personal well-being. The Life Path Support Worker, will be a part of a team that provides services designed to enable residents to achieve change and growth.
Duties and Responsibilities:
- Provide one-on-one or group support based on individualized service plans.
- Assist clients with daily living skills (e.g., cooking, budgeting, personal care as required).
- Support clients in engagement with employment, education, recreation, and community activities.
- Provide supportive, non-clinical counselling to help clients cope with stress, anxiety, life transitions, and interpersonal challenges.
- Encourage personal growth, self-advocacy, independence, and decision-making through developing coping strategies, emotional regulation skills, and self-awareness.
- Use active listening, solution-focused approaches, and strength-based techniques to support client goals.
- Help clients identify and work through barriers that affect their well-being and quality of life.
- Monitor changes in behaviour, mood, or mental health and communicate concerns to supervisors or clinical teams when appropriate.
- Facilitate discussions related to problem-solving, conflict resolution, and healthy relationships.
- Maintain accurate documentation, progress notes, and incident reports.
- Collaborate with families, case managers, health professionals, and community partners.
- Promote a safe, inclusive, trauma-informed, and respectful environment.
Qualifications
- Diploma or certificate in, Counselling, Social Services, Community Support Work, Psychology, Counselling, Mental Health, or a related field (or equivalent experience).
- Demonstrated counselling or supportive communication skills, including active listening, de-escalation, and strength-based approaches.
- Strong interpersonal and documentation skills.
- Ability to work independently and as part of a collaborative team.
- Valid driver’s license.
- First Aid/CPR
- Compassion, patience, and commitment to person-centered and trauma-informed care.
- Willing to work evenings, and weekends
- Must submit a criminal record check, child abuse registry check and drug testing as per FRCN By-law 2004-01, sec. 8.01);
